Were searching for a Home Health Registered Nurse (RN) to provide professional intermittent skilled nursing and individualized, family-centered care to pediatric patients/families through the use of the nursing process and clinical skills in accordance with standards and home healthcare policies and procedures.

ESSENTIAL FUNCTIONS:

  • Conducts a complete individualized age-specific patient/family assessment according to defined home healthcare scope and nursing practice standards, nursing diagnoses, or protocols. (Individualized refers to the patients developmental, cognitive, cultural, environmental, psychosocial, spiritual, and educational/family influences and physical/medical care needs.)
  • Collects and documents data related to patients individualized care. This will be completed either within 48 hours of the referral or on the physician ordered start of care date.
  • The initial assessment reflects information received from the referral source, interviews with caregivers and other team members and direct physical assessment of the patient and the environment.
  • Demonstrates age-specific competency in assessment of patients by the ability to obtain and interpret information in terms of the patient needs, knowledge of growth and development, and understanding of the range of treatment needed by these patients.
  • Assesses patient/family need for education and uses an individualized approach in delivery style.
  • Assesses family expectations for, and involvement in the assessment and continuous care or treatment of the patient.
  • Identifies nursing diagnoses, collaborative problems, and/or protocols for each patient, based on assessment, at the time of admission.
  • The initial patient assessment will be documented within 1 day of the start of care.
  • Incorporates individualized patient/family plan of care guidelines/protocols to meet identified needs. (Plan of Care includes current and anticipated actions focused on care, education, discharge needs.)
  • Utilizes assessment data from the healthcare team to individualize appropriate goals and actions for the patient, including collaboration with the family and the healthcare team.
  • Utilizes individualized assessment data to develop the patient/familys plan of care.
  • Formulates goals and outcomes based on assessment data.
  • Revises plan of care based on patients condition, interdisciplinary input, and family needs.
  • Demonstrates ability to prioritize patient interventions based on assessments/reassessments.
  • Implements and evaluates nursing actions in accordance with the individualized patient/family plan of care and patient condition/needs.
  • Oversees that the plan of care and goals are coordinated, implemented, evaluated, and revised as needed. Identifies the need for interdisciplinary involvement/outside resources and follows through. Uses appropriate resources (Nurse Manager, Pharmacist, Physician, Respiratory Therapist, Product / Purchasing Specialist) to ensure that clinical outcomes are achieved.
  • Incorporates critical thinking into the delivery of clinical skills and in analyzing patient response to orders, tests, labs, medications, and treatments.
  • Demonstrates clinical competence in administering interventions according to identified standards and age specific needs.
  • Provides and/or coordinates with other disciplines patient/family education according to identified care needs, considering assessment of patient/familys readiness to learn.
  • Verifies that caregivers feel competent and independent to perform all necessary functions to care for the child, and offers ongoing support with each nursing visit.
  • Responds appropriately to emergency patient needsdemonstrates the ability to function independently in the home setting.
  • Documentation reflects accurate assessment/evaluation, modification, and patients response to therapy, changes in condition, and interventions.
  • Entries in the medical record follow organizations documentation policy (i.e., date, time, use of appropriate abbreviations, and legibility).
  • Documentation is completed within established timeframes including, but not limited to: Initial Assessment form, Client Rental Equipment Agreement, CHHC Parent Handbook and Privacy Receipt, Appropriate Care Plan with Goals, Advanced Beneficiary Notice (as appropriate), Medication Profile, Appropriate Flow Sheets and Assessment Forms, Teaching Checklists and Growth Charts when indicated.
  • Reviews and updates Care Plans, Orders, Medication Profile to accurately reflect the plan of care.
  • Ensures that discharge instructions clearly state patient care needs after discharge, and Discharge Summaries accurately reflect current and ongoing services
  • Accepts assignments to share knowledge, provide clinical support, and assist new staff through the orientation process in accordance to all designated policies and procedures.
  • Professionally seeks opportunities to help teammates during work day as needed.
  • Accepts and completes other assigned tasks in a timely and professional manner.
  • Participates in the on-call rotation as specified by Nurse Manager.

KNOWLEDGE, SKILLS AND ABILITIES

  • Must be able to perform a variety of duties, often changing from one task to another of a different nature, without loss of efficiency or composure, and be able to perform under stressful conditions when confronted with multiple requests, demonstrating prioritizing skills
  • Basic Computer Skills
  • Oral and written communication skills (bilingual skills preferred)

EDUCATION AND EXPERIENCE

  • Diploma, Associates, or Bachelors degree from an accredited school of nursing required
  • Minimum two years nursing experience required
  • Experience with pediatric patients preferred
  • Experience in homecare environment preferred
  • CERTIFICATIONS/LICENSURE REQUIREMENTS:
  • Current valid Nebraska RN license/RN Compact license required
  • Current and valid Basic Life Support (BLS) certification required

SPECIAL REQUIREMENTS:

  • Program Specialty certification or nursing leadership certification preferred, within one year of eligibility
  • A valid drivers license and an acceptable motor vehicle record/driving record required
